What this test is

Iron Studies are the next step after a CBC has flagged low haemoglobin. Serum iron alone fluctuates by the hour; ferritin tells you how much iron is stored. Low ferritin (under 30 ng/mL) confirms iron deficiency — the most common cause of anaemia in India, especially in women of reproductive age. The panel also rules out iron-overload conditions like haemochromatosis.

This panel reports 4 parameters

  • Serum Iron
  • TIBC (Total Iron Binding Capacity)
  • Transferrin Saturation
  • Ferritin

When you need this test

  • Confirmed low haemoglobin on CBC
  • Heavy menstrual bleeding
  • Pregnancy
  • Fatigue with brittle nails or hair fall
  • Restless legs
  • After bariatric surgery or chronic GI bleeding

How to prepare

Fasting required — 8 hours, water OK
  • Fast for 8 hours — only water during the fast.
  • Skip iron supplements for 24 hours before the test, since fresh supplement-iron can falsely raise serum iron.
  • Morning slot preferred — serum iron is highest in the morning.

FAQs

  • Why fast?
Iron levels vary by 30–40% through the day depending on meals. Fasting plus a morning draw makes results comparable over time.
  • Why is ferritin the most important number?
Ferritin reflects stored iron and is the most reliable single indicator of deficiency.
  • How quickly will supplements raise my numbers?
Haemoglobin rises by about 1 g/dL in 3–4 weeks. Ferritin takes 3–6 months to refill stores.
  • Should I take iron with food?
Take it on an empty stomach with a glass of orange juice if tolerable — Vitamin C boosts absorption. Avoid tea, coffee, and milk within 1 hour.
